Incident Reporting

Mineralogical Society Incident Report Form

This is the form in which to report behaviour at a Society event which is contrary to our Society Code of Conduct. A form for reporting other, general feedback about our events is available here. For information, a link to our Disciplinary Procedure is also provided here.

——————————————————————————————————————————————————————————————
What happens when you complete this form? This form will be saved on a confidential Mineralogical Society server. A message will be sent immediately to the Executive Director who will decide on action to be taken. He will contact the affected person to ensure their safety. He will contact the event organizers to agree actions.  Please contact him via kevin@minersoc.org or via telephone if you wish to discuss what happens to your report.

The Society will then follow its Disciplinary Procedure. The report will be kept on confidential file until the disciplinary process is complete and for a further period of three years (to allow for appeals etc.). It will then be destroyed.
